# Schools and tutoring: enrollment and permission forms

Tutoring centers, music and language schools, camps and private schools can send enrollment agreements and permission forms for a parent or guardian to sign on their phone, with no account, app or fee for the family. With Attest, each family's form is end-to-end encrypted and sealed into a copy the school and the family both keep, which shows which version each parent approved with their own passkey, and when, and anyone can verify it for free.

## Forms that fit

- **Enrollment agreements:** the program, the schedule, the fees and payment dates, and the cancellation, refund and make-up lesson policy.
- **Permission for one activity:** a field trip, a competition, an off-site class, or using a student's photo on your website (see [photo and model releases](https://attest.datahubz.com/use-cases/photo-and-model-releases)).
- **Program agreements:** a code of conduct or a technology use agreement that the family confirms.
- **Agreements with tutors and instructors** who work for you as contractors (see [client contracts and statements of work](https://attest.datahubz.com/use-cases/client-contracts-and-statements-of-work)).

Keep each form to the agreement or the permission itself. Each form is a PDF of up to 8 MB that you upload.

## What a form asks each parent for

Each parent fills in the fields you place for them on the PDF, and there are five kinds: **Signature**, **Initials**, **Date signed**, **Full name** and **Text**. Signature, initials, date signed and full name are always required. A text field carries a label of up to 60 characters telling the parent what to enter, such as "Student's class", and you choose whether it is required.

For a yes or no permission, such as using a student's photo on your website, write the question in the form and place a **Text** field beside it, marked **Required** and labelled with what to enter, for example "Photo use: type yes or no". Each parent's answer is part of what their passkey signs, and the signed copy shows it on the page. For terms every family must accept, such as a code of conduct, place an **Initials** field beside each section, so the parent initials each one before signing.

## Who signs: the parent or guardian

When the student is a minor, the parent or guardian signs, with their own email address. Name the student in the form. An adult learner signs their own enrollment agreement. If your policy needs both parents, add both as signers: one document can have up to 20 signers, they can sign in any order, and the form completes only when every one of them has signed.

Parents sign in the browser, on a phone or a computer, with nothing to install. They create no account, set no password and never pay. They approve with a **passkey**, a cryptographic key only the signer controls, unlocked with Face ID, Touch ID, Windows Hello or the device PIN. The first time a parent signs with Attest, they set up the passkey from their link, and you release the encrypted form to them from Attest's email, approving with your passkey.

## One family, one form

Send each family its own document. Each one is encrypted only to that family's signers and your school, so no parent sees another family's form, fees or email address, and each family's form is its own signed record, with its own fingerprint and anchor. Attest encrypts every form in your browser before upload and stores it only as ciphertext it can't read.

Attest does see the information around a form, such as its title, file name and the signers' emails and names, so use titles like "Enrollment, fall term, family 12" rather than the student's name. Choose **Private** under Verification: only the family and your school then see the certificate, which otherwise shows anyone with its link the title and each signer's name and signing time, among other details.

## Deadlines that work for trips and terms

- **Expires (optional):** set it to the day before the trip, or the last enrollment day, up to a year ahead. If the form isn't signed by then, it closes, no one can sign it, and every party is emailed; a parent who opens the link afterwards sees "This document expired".
- **Reminders:** every 3 days by default, or every day, every 2 days or every week, at most 5 times, and each reminder states the expiry date.
- **Remind:** emails the link again to a family whenever you choose; it restarts the automatic reminders' clock and doesn't count toward the 5.
- **Waiting on others:** this dashboard tab lists the forms waiting on families, so you can see at a glance who hasn't signed; a form waiting for you to release it to a parent who is new to Attest is under **Action needed**.
- **Message (optional):** a note shown in the invitation email and on the signing page, such as "Please sign by Friday for the museum trip."

## When a question comes up later

Months later, a family may ask what the cancellation policy said, or whether they agreed to a trip. The signed copy answers it:

- **Which policy.** The parent's passkey signs a statement that names the form's **fingerprint** (a SHA-256 hash, a short code calculated from the file's contents) and everything they entered. Last term's cancellation policy and this term's have different fingerprints, so the copy shows which one the family accepted.
- **Before the trip.** The signing time is part of what the passkey signs, and the completed form is anchored on Horizen, a public blockchain, so the permission's date doesn't rest on your own records alone.
- **Unchanged.** Change one byte of the form, of any signer's entries or of the signed pages, and verification fails.

The school and the family hold the same sealed PDF, which carries its own evidence, the original document and an open-source verifier inside it. Anyone can check it for free at attest.datahubz.com/verify, with no account, and the verifier inside the PDF runs in any browser, even if Attest is gone. A parent can open the signed copy from their link for 30 days after completion, and after that by signing in with their email address.

## Plans for a tutor, a center or a school

- **A tutor with a few families:** your first 2 documents are free, in total, then the Signing plan is $5 a month for 20 documents, and you can cancel any time.
- **A center or school enrolling many families:** Pro is $49 a month with 10,000 records a month, each document you send counting as one, plus Attest's other tools.
- **Several staff who send forms:** an organization plan covers every member who joins, and each member gets the better of their own plan and the organization's. Organization plans are arranged with DataHubz.

Parents and guardians never pay to sign, on any plan.

## How to send a permission form

1. Open **Documents** in the menu (it reads **Agreements** on accounts that use every Attest tool) and click **New agreement**.
2. Give it a plain **Title**, such as "Museum trip, family 12", click **Attach the document to be signed** and choose the PDF.
3. Under **Signers**, type the parent's or guardian's email address; click **\+ Add signer** to add a second parent.
4. Under **Verification**, choose **Private**. Set **Expires (optional)** to the day before the trip, and choose the **Reminders** interval.
5. Click **Next: place the fields**, place a signature field and a date signed field for each parent, plus a **Text** field for each question the form asks, such as the photo permission above, and click **Create and send**.
6. When a parent who is new to Attest has set up their key, Attest emails you. Click **Unlock it for …** in that email (or open the agreement in Attest), confirm, and approve with your passkey: your browser releases it to them, never Attest.
7. When the form is signed, click **View the signed copy**, then **Download signed copy** for your records.

## Questions people ask

Can parents sign school forms electronically?

Electronic signatures are recognized for most agreements in the United States (the ESIGN Act and UETA) and the European Union (eIDAS). Some documents, such as wills or certain notarized deeds, have special rules in many places: check yours. Under ESIGN, for a transaction in or affecting interstate or foreign commerce, a signature, contract or other record "may not be denied legal effect, validity, or enforceability solely because it is in electronic form" (15 U.S.C. 7001(a)(1)). This is general information, not legal advice.

Can a student sign their own form?

An adult learner can sign their own enrollment agreement. When the student is a minor, the parent or guardian signs, and the student is named in the form.

Do parents need an account or an app?

No. Parents open their private link in the browser and set up a passkey, with no account to create, no password and nothing to pay. The first time a parent signs with Attest, you release the encrypted form to them with your passkey.

Can both parents sign the same form?

Yes. Add both as signers; they can sign in any order, and the form completes when both have signed. Each receives the same sealed copy.

How do we send a form to a whole class?

Send each family its own document, so each family sees only its own form, and each form is its own signed record. The Signing plan covers 20 documents a month, and Pro covers 10,000 records a month for schools that send more.

What if a parent hasn't signed before the trip?

Attest reminds them automatically, and Remind emails the link again whenever you choose. If you set an expiry date and it passes, the form closes, no one can sign it, and every party is emailed.
